1. Have you ever tried to, or felt the need to, cut down on your smoking? Yes No
2. Do you ever get annoyed when people tell you to quit smoking? Yes No
3. Do you ever feel guilty about smoking? Yes No
4. Do you ever smoke within one-half hour of waking up? Yes No
If you answered yes to one or more questions, you might want to consider quitting smoking. Many people are not aware of all the ways that smoking can affect their lives. The following information and exercises will help you to identify difficulties you might be facing. Your responses to assessment questions might alert you to problems that you have not thought about before. Please keep in mind that tobacco consumption in any amount or form could have harmful effects on your health.
